Client Acknowledgement Required

I understand that intuitive readings, psychic mediumship sessions, and animal communication readings are being provided by Melissa Landolt, psychic medium, at my request.

I understand that Intuitive coaches and psychic mediums do not diagnose mental health disorders, or medical conditions, prescribe substances, nor interfere with the treatment of a licensed medical and/or mental health professional. I understand that Melissa Landolt is not a licensed physician and that intuitive readings, psychic mediumship sessions and animal communication readings are complementary to and separate from licensed medical services. Always seek the advice of a qualified healthcare provider if you have any questions regarding a medical or health condition.

I understand that Melissa is not a veterinarian and that the information Melissa receives as a pet intuitive should not replace that of a professional animal care provider.

Information provided by Melissa Landolt should not be relied upon as a substitute for advice, programs, or treatment that you would normally receive from a licensed professional, such as a lawyer, doctor, psychiatrist, psychotherapist, veterinarian, or financial adviser.

I assume sole responsibility for my own physical and mental health and for the results of any sessions provided by Melissa Landolt that may affect me in any way.

Clients under the age of 18 must have written permission from a parent or guardian. All children under the age of 16 must be accompanied by an adult.

Confidentiality: All sessions are confidential unless you are in immediate danger of hurting yourself or others, or you are committing child/elder abuse.

Cancellation and refund policy for individual sessions and packages:

Unless there is a last-minute emergency, appointments canceled less than 48 hours in advance are considered a missed appointment and I will be charged in full for the service. Sessions are non-refundable after a reading has been completed.

Packages are non-refundable. Appointments canceled less than 24 hours in advance will be considered a missed appointment and will be subtracted from the total number of sessions in the package.

All information I received from Melissa Landolt is accepted with full knowledge that any action I took as a result of the information received is my complete responsibility.

I have read, understood, and accepted the informed consent as shown on Melissa Landolt’s website. I hereby acknowledge and affirm my understanding and agreement with the above.
